Blue Chip Contract

Husky Educational Technology Assessment Program (HETAP) 

Memorandum of Agreement (Agreement)

between

Roles and Responsibilities:

  • University of Connecticut agrees to
    • Administer the Connecticut Statewide Husky Educational Technology Assessment Program to educators at the School/District as follows: 
    • Administer and score _____ Level I assessments; ($81.00 per participant assessment)
    • Administer and score _____ Level II assessments ($216.00 per participant assessment)
    • Administer and score _____ Level III assessments ($184.50 per participant assessment)
    • Maintain the Husky Educational Technology Assessment Program web site containing:
    • Study Guides, Competencies assessed; assessment schedules and HETAP contact people; 
    • Those passing the assessment and signing consent forms will have their names and schools listed on the web site;
    • Maintain an asynchronous web-based discussion list allowing educators across the state to be able to discuss educational technology issues with one another.
    • Store the record of those educators completing the educational technology assessment in an established electronic database.
    • Inform all educators completing the assessments of their results.
    • Mail certificates of accomplishments to educators passing the assessment.
    • Maintain a regular interval of statistical checks to maintain reliability and validity estimates.

Note: the UConn team retains all rights to the Husky Educational Technology Assessment Project and these assessments may not be used without written permission from Drs. Kulikowich and Brown. Furthermore, the HETAP team retains the rights to publish research manuscripts related to the development and results of this assessment without revealing names of individuals, school districts or organizations. All manuscripts will contain an acknowledgement of the support of the Connecticut State Department of Education.

 

  • School/District agrees:

    • To pay the University of Connecticut a total of $__________ upon execution of this Agreement for the implementation of a statewide educational technology teacher assessment system. Funds are payable to the University of Connecticut.
    • To provide a list of educators authorized to complete the HETAP at each level to be used as a count against the total participants contracted.
    • That this is a fixed-price effort and no reporting of expenditures is required

The contract may be extend by written agreement of both parties.

This agreement is acceptable to both parties and the terms will be accepted as binding for the services herein described.
